All "Affected Persons" (which includes all players born in 2009 or earlier, all team officials, and all volunteers) must provide proof that they have received the completed series of an Accepted COVID-19 Vaccine by October 17, 2021 or earlier. Head Coaches and Trainers will be making arrangements with each team to collect the required proof.
The
OWHA Vaccination Policy sets out detailed requirements for medical exemptions. Any Affected Person who wishes to begin the accommodation request process must submit an OWHA
Medical Exemption Request form with the necessary documentation to the Twisters at
[email protected] and the OWHA at
[email protected] as soon as possible.
